How a Referral Works

A straightforward, clinician-to-clinician process designed to make discharge transitions as smooth as possible for your clients.

Facility Refers a Client

Your treatment team identifies a client approaching discharge who is ready for structured sober living. You contact our admissions coordinator directly to discuss fit and availability.

Joint Clinical Review

We review the client's clinical summary, discharge plan, and outpatient schedule together. Our team confirms that Anchor Point's structure aligns with their ongoing treatment needs.

Outpatient Coordination

Residents continue their outpatient program — IOP, PHP, individual therapy, or medication management — while living at Anchor Point. We coordinate transportation and scheduling to remove barriers.

Seamless Transition

On discharge day, the client moves directly into their room at Anchor Point. We handle the handoff with your team to ensure continuity of care and a smooth, supported transition.

Ongoing Communication

We maintain open communication with referring facilities throughout the resident's stay — sharing progress updates, flagging concerns early, and collaborating on step-down planning.

Partnership FAQ

Do residents need to be enrolled in outpatient services before moving in?

Yes — we require that all residents have an active outpatient treatment plan in place at the time of move-in. This may be an IOP, PHP, individual therapy, or medication-assisted treatment program. We work closely with your team to confirm this before the transition.

Can Anchor Point help coordinate outpatient placement if one isn't yet established?

In some cases, yes. We have established relationships with outpatient providers and can assist with referrals when needed. Reach out to our admissions coordinator to discuss.

How quickly can a client be placed after discharge?

With advance notice, we can typically accommodate a placement within 24–72 hours of discharge. We encourage facilities to reach out as early as possible in the discharge planning process to ensure availability.

What happens if a resident's outpatient needs change during their stay?

We stay in close contact with referring clinicians and outpatient providers. If a resident's clinical needs escalate or change, we communicate immediately and collaborate on the appropriate next steps — including step-up to a higher level of care if needed.

Is there a formal referral agreement required?

We welcome both formal partnership agreements and informal referral relationships. Many of our facility partners begin with a single referral and formalize the relationship over time. Contact us to discuss what works best for your organization.
